Innovative Approaches to Address Opioid Addiction and Chronic Pain Management For individuals suffering from chronic pain and the healthcare providers treating them, reducing reliance on prescription opioids has been a complex challenge. To tackle the opioid crisis, Vessel Partners collaborated with MedStar Health, a prominent healthcare system in the Washington D.C. area, on an AHRQ-funded initiative aimed at equipping patients and healthcare professionals with new resources. The outcome is TAPR-CPM — a suite of applications that seamlessly integrate with various electronic medical record (EMR) systems and offer features such as: - Pain monitoring leveraging Computer Adaptive Technology (CAT) PROMIS measures - A body map for pinpointing pain locations - Sentiment analysis to evaluate mood and overall well-being - Tracking of non-opioid pain management strategies - Visual trends over time regarding pain and treatment approaches - FHIR and SMART on FHIR data APIs to enhance EHR interoperability - Clinical Decision Support The Clinical Decision Support Tool within TAPR aids physicians and clinicians by providing essential information for developing evidence-based tapering strategies designed to minimize withdrawal symptoms and reduce the likelihood of tapering failures. By utilizing specific patient data regarding medications and individual needs, the app offers tailored recommendations for gradual dosage reductions at appropriate intervals based on a patient's morphine milligram equivalent (MME). Adhering to optimal tapering rates is crucial for success, and TAPR notifies clinicians if they deviate from established evidence-based protocols. Furthermore, the app recommends alternative non-opioid pain management options for prescription and strategies for alleviating withdrawal symptoms. Clinicians can track patient advancements over time and receive alerts regarding significant changes in the patient's patient-reported outcome (PRO) scores. During follow-up appointments, doctors have the flexibility to adjust the tapering process based on the data generated by the patient.

Enhancing Patient-Reported Outcomes in Remote Patient Monitoring The contemporary healthcare system largely relies on discrete patient visits, which offers clinicians a restricted view of their patients' health in real-world settings. Remote monitoring of Patient-Reported Outcome Measures (PROMs or PROs) can provide clinicians with valuable insights into a patient's ongoing health and recovery, but challenges with interoperability and usability have slowed its adoption. To address this, we partnered with researchers from the University of Minnesota’s Carlson School of Management and the Institute for Health Informatics, along with Fairview Health System/HealthEast Kidney Stone Institute, to develop more effective methods for collecting PROs in remote patient monitoring. This collaboration led to the creation of an easy-to-use web and mobile application called PRISM, aimed at improving PRO adoption. PROMIS has already been successfully utilized in clinical settings and won the AHRQ Step Up App Challenge. Our development process employed a four-step human-centered design approach: 1) We interviewed clinicians and stakeholders to capture their needs. 2) We spoke with patients to gather their preferences. 3) Our team developed an interactive prototype and conducted user testing with actual patients in clinical settings. 4) We refined the prototype through four rounds of testing based on both quantitative and qualitative feedback, confirming that it was user-friendly and aligned with the users' objectives. "This app is a groundbreaking tool that will promote value- and person-centered care by prioritizing patient-generated information in their treatment.
